Self-Pay Rates
$100 for a 45 minute individual session
$115 for a 60 minute individual session
$130 for a couples or family session
Full charge for late cancellations that are requested without a 24-hour advance notice, applicable to both clients who self pay or use insurance. This means that you will pay cost of session, either contracted by insurance company or self pay agreement.
Based on daytime availability, I also offer sliding scale for people who are experiencing financial hardships.
Insurance: Out-of-Network Coverage
Together, we can manage out of network coverage. You may receive partial reimbursement from your insurance company if your plan covers services with “out-of-network providers.” I can provide a monthly receipt, which you can submit to the insurance carrier for reimbursement.
When you call your insurance company, you may wish to ask:
- Does your plan cover “out-of-network outpatient mental health services”?
- What is your annual deductible and has it been met?
- What is your copayment per visit?
- What is the maximum number of sessions allowed per year?
- What is the “reasonable and customary fee” that the company uses as a basis for reimbursement? To determine the reasonable and customary fee, they may ask for the provider’s license type, taxpayer ID and zip code.
Insurance: In-Network Coverage
Until April 2nd 2024 I am an “in-network” provider with the following EAP and insurance companies. After April 2nd I will be transitioning to out-of-network provider status. Client covered under these insurance are only responsible for the co- payments:
- CareSource
- Cigna
- Medical Mutual
- United Behavioral Health
